Health-Related Information
To help determine whether our referral service may be relevant to your situation, we ask general, self-reported questions about whether you are currently able to work and how long your situation is expected to last. We do not ask you to identify a diagnosis or condition, and we do not collect or review medical records.
Some state privacy laws treat information about a person's health or physical or mental condition as sensitive personal information or as consumer health data. We treat the information described above accordingly:
- We collect only what is needed to determine whether we may be able to refer you to a provider
- We do not use it for any purpose unrelated to that referral and the communications you consented to
- We do not use it to build advertising profiles or to target advertising based on health status
- You may ask us to delete it at any time using the methods described in the Consumer Rights section below
Washington, Nevada, and Connecticut consumer health data. Washington's My Health My Data Act, Nevada SB 370, and the Connecticut Data Privacy Act give residents of those states specific rights regarding consumer health data. Depending on how those laws are applied, the self-reported answers you give us about your ability to work may be treated as consumer health data.
If you are a resident of one of those states:
- We collect this information only with your consent, which you give when you submit our form or provide the information by phone.
- We use it only to determine whether our referral service may be relevant to your situation and to connect you with the independent providers described in the Sharing of Information section below.
- We do not use it for targeted advertising, and we do not sell it separately from the referral described above.
- You may withdraw your consent, request a list of the third parties with whom we have shared it, or request that we delete it, by emailing privacy@pickmydisability.com or calling 1-833-418-3637. We will confirm deletion and notify our processors, and we will respond within the timeframe required by the applicable law.
- You may also submit these requests through our Do Not Sell or Share My Personal Information page.
We do not use geofencing around any health care facility.
